Attention! Student Athletes and/or Camp Participants

In
order to participate in school sports or attend camps, state
regulations require that you have a sports or participation physical.
Sports physicals should be done well before your sport or camp begins.
The physical exam enables your healthcare provider to update your
health profile.
To save you time and to serve you better, we ask that
you bring:
- Your medical history form provided by your school or camp
(please have the form filled out prior to your arrival)
- A list of all over-the-counter medications you are taking, including herbs and vitamins
- A list of any prescription medications and their dosage
During the Physical Examination, your healthcare provider will:
- Record your height and weight
- Take a blood pressure and pulse
reading
- Test your vision
- Check
your heart, lungs, abdomen, ears, nose, and throat
- Evaluate your posture,
joints, and nervous system
If,
while playing sports, you notice a shortness of breath, dizziness,
muscle pain, cramping, or anything that causes you to feel
uncomfortable, please mention this immediately to your coach, parent,
or athletic trainer. It may be a sign of a more serious condition that
should be monitored or treated.
